What determines which monster attacks first aside from speed?

I understand that when the speed of one monster is higher than another, that monster goes first. Basically, a monster with four-speed will attack before a monster with three-speed. what I am wondering is which monster attacks first when they both have the same speed (they both have three-speed for example). How is this determined? How does it vary by rounds?
