Esta semana no fue la excepción, además de que estas salidas me sirven para despejar un poco la mente cuando termino las consultas, ya que normalmente cuando veo pacientes a domicilio es porque están bastante descompensados y, por consiguiente, debo manejar un entorno familiar bastante denso.
A muchos colegas no les gusta realizar visitas a domicilio por este factor, el hecho de que usualmente son muchos familiares con los que hay que comunicarse, explicarles a cada uno y tratar de entender una dinámica que siempre es bastante estresante, justamente porque el paciente usualmente está en malas condiciones.
En geriatría no podemos sencillamente omitir estas visitas domiciliarias, porque gran porcentaje de nuestros pacientes posee síndrome de Inmovilidad, por múltiples razones, es decir están en cama o sillas de rueda, lo que hace que sea difícil su traslado a centros de salud o consultorios, por lo que somos los médicos geriatras los que nos vemos obligados a trasladarnos a sus lugares de residencia.
Whenever I go out to see a patient at home, I take the opportunity to take my parents with me to invite them for a coffee and a snack, and it should be noted that it is my dad who takes me because I do not know how to drive 🫠.
This week was no exception, besides the fact that these outings help me to clear my mind a bit when I finish my consultations, since normally when I see patients at home it is because they are quite decompensated and, consequently, I have to manage a rather dense family environment.
Many colleagues do not like to make home visits because of this factor, the fact that there are usually many family members with whom you have to communicate, explain to each one and try to understand a dynamic that is always quite stressful, precisely because the patient is usually in poor condition.
In geriatrics we cannot simply omit these home visits, because a large percentage of our patients have immobility syndrome, for multiple reasons, i.e. they are in bed or wheelchairs, which makes it difficult to transfer them to health centers or offices, so we geriatricians are the ones who are forced to move to their places of residence.
A mí no es que me desagrade del todo, es un trabajo que hago igual con amor porque sé que es una persona que me necesita, yo me tomo un tiempo considerable en mis consultas porque me gusta explicarle bien a los familiares los pasos que vamos a seguir en el plan de trabajo y aprovecho para intentar entender un poco el entorno familiar, ver que tan factible es llevar a cabo el plan y saber cuan preocupados están realmente por el enfermo.
Sin embargo, acepto que es un trabajo agotador porque una cosa es explicarle algo a una persona que acompaña al paciente en un consultorio y otra muy distinta explicárselo a 3 o hasta 6 personas a la vez, además de responder todas las interrogantes de todos a la vez, incluyendo los que están afuera y me llaman por teléfono para que les explique también 😪.
Por otro lado, estando ahí, he tenido que ver infinidad de situaciones desagradables, como discusiones entre hermanos por ver quienes van a realizar tales cosas en pro del paciente, quienes cuidaran de él, o cuando lo harán, quienes harán los gastos económicos, incluso, he tenido que percibir malos tratos hacia el paciente, lo cual, por cierto, es lo más desagradable de todo.
Todas estas son situaciones bastante estresantes que se anexan a la evaluación médica que debemos realizar y es la razón del por qué a muchos colegas no les gusta realizarlas en los domicilios.
It is not that I dislike it at all, it is a job that I do with love because I know that this is a person who needs me, I take a considerable amount of time in my consultations because I like to explain to the family members the steps we are going to follow in the work plan and I take the opportunity to try to understand the family environment, to see how feasible it is to carry out the plan and to know how concerned they really are about the patient.
However, I accept that it is an exhausting job because it is one thing to explain something to one person accompanying the patient in an office and quite another to explain it to 3 or even 6 people at a time, in addition to answering all the questions of everyone at once, including those who are outside and call me on the phone for me to explain them too 😪.
On the other hand, being there, I have had to see countless unpleasant situations, such as arguments between siblings to see who will do such things for the patient, who will take care of him, or when they will do it, who will make the economic expenses, even, I have had to perceive mistreatment towards the patient, which, by the way, is the most unpleasant of all.
All these are quite stressful situations that are added to the medical evaluation that we must perform and that is the reason why many colleagues do not like to perform them at home.
El hecho es que terminando este paciente que vi ayer, que está bastante delicado de salud, le dije a mi mamá que necesitaba urgente un café y algo dulce para apaciguar mi estrés, así que nos sentamos los 3 a tomar nuestros deliciosos café que ya estamos acostumbrados a comprar en Johnys Bakery, ellos ya nos conocen y saben como nos gusta a los 3 el café, a mi un "marrón", mi mama un café con leche sin espuma y mi papá uno normal con leche.
Además de eso, esta vez pedí un mini "Pie de limón", solo para calmarme un poco, el cual me encantó porque la crema estaba super suave, la galleta crocante y el relleno con un dulzor acorde, ya saben que no me gustan los postres demasiado dulces, su precio fue 2$.
Cuando ya nos íbamos, notamos que estaban vendiendo unas "Fresas con crema" que anexaron al menú, así que terminamos la tarde comiéndonos una cada uno, estaban bien, las fresas estaban frescas y tampoco estaba todo tan dulce, la cantidad supongo que era acuerdo al precio, por las 3 fueron 5$ ya que estaban en promoción, lo cual me pareció bien (pd.no sé que pasó con el resto de las fotos, por eso solo tengo esta que les muestro 🥲).
Como verán, yo soy muy apegada a mis padres y siempre intento llevarlos a algún lugar para que salgamos de la casa, de hecho creo que el no aprender a manejar es una estrategia que inconscientemente he hecho para mantener activo a mi papá y a mi mamá que es mi asistente y atiende mis llamadas, ambos están jubilados de sus trabajos, yo les dije que lo hicieran cuando yo me gradué de médico porque su salud se estaba deteriorando.
The fact is that finishing this patient that I saw yesterday, who is quite delicate in health, I told my mom that I urgently needed a coffee and something sweet to appease my stress, so the 3 of us sat down to take our delicious coffee that we are already used to buy in Johnys Bakery, they already know us and know how we all 3 like coffee, me a "brown", my mom a coffee with milk without foam and my dad a normal one with milk.
Besides that, this time I ordered a mini "Lemon Pie", just to calm down a little, which I loved because the cream was super soft, the cookie crunchy and the filling with a matching sweetness, you know I don't like desserts too sweet, its price was 2$.
When we were leaving, we noticed that they were selling some "Strawberries with cream" that they added to the menu, so we finished the afternoon eating one each, they were good, the strawberries were fresh and not too sweet, the quantity I guess was according to the price, for the 3 were 5$ since they were on promotion, which I thought was good (pd.I do not know what happened with the rest of the photos, that's why I only have this one 🥲).
As you will see, I am very attached to my parents and always try to take them somewhere to get us out of the house, in fact I think not learning to drive is a strategy I have unconsciously done to keep my dad and my mom who is my assistant and takes my calls active, they are both retired from their jobs, I told them to do it when I graduated as a doctor because their health was deteriorating.
A fin de cuentas soy Geriatra, así que quiero mantener a mis padres que tienen 69 y 73 años, lo más activo posibles durante el mayor tiempo posible, para así asegurarles calidad de vida en su vejez y, a la vez, ayudarme a mí con mi trabajo, que termina siendo igual en beneficio de ellos, porque soy quien provee los mayores gastos económicos de la casa.
En fin que solo quería contarles un poco de cómo es nuestra dinámica familiar y como intento mantener un equilibrio entre todo para no recargarme yo sola de responsabilidades e intentar mantener el estrés del trabajo en su lugar, mientras les mostraba estas provocativas fotos 😅.

At the end of the day I am a Geriatrician, so I want to keep my parents, who are 69 and 73 years old, as active as possible for as long as possible, in order to assure them quality of life in their old age and, at the same time, help me with my work, which ends up being of equal benefit to them, because I am the one who provides the major financial expenses of the house.
