Siempre que conozco un paciente nuevo, me preguntan ¿Por qué decidiste especializarte en geriatría? Y es que por alguna razón la mayoría asocia que a adultos mayores sólo los pueden atender personas de mayor edad y me da mucha risa pensar que, si supieran todo lo que hay que saber para atenderlos, si empezamos a estudiar a esas edades, entonces tristemente nunca lo lograríamos.
Esta semana 2 de mis pacientes me han dado 2 regalos de navidad y creo que han sido de los mejores regalos que me han dado en mi vida porque son, nada más y nada menos, que libros escritos por ellos mismos.
Aquí es donde esto entraría a formar parte de esa respuesta a la pregunta que hice arriba del por qué decidí estudiar Geriatría, quiero decir ¿Cuántas especialidades pueden darse el lujo de decir que entre sus pacientes hay escritores de libros? ¿Verdad que son pocas?
Ya que mis pacientes tienen edades más avanzadas que las edades comunes, evidentemente han logrado alcanzar más metas que otros más jóvenes, he conocido a doctores en distintas ciencias, magísteres, philosophus doctors, exmilitares, personas que trabajaron en clandestinidad durante muchos años, ya que vivieron en épocas difíciles en Venezuela como durante la dictadura castrense, personas que sobrevivieron a catástrofes naturales como terremotos o inundaciones donde sobrevivieron sólo unos pocos, personas que llegaron como primeros inmigrantes a ciertas ciudades, los primeros en realizar ciertas actividades, etc.
Whenever I meet a new patient, I am always asked why did you decide to specialize in geriatrics? And for some reason most people associate that only older adults can be cared for by older people and it makes me laugh to think that if they knew everything there is to know about caring for them, if we started studying at that age, then sadly we would never make it.
This week 2 of my patients have given me 2 Christmas gifts and I think they have been some of the best gifts I have ever been given in my life because they are, nothing more and nothing less, than books written by themselves.
This is where this would come into part of that answer to the question I asked above as to why I decided to study Geriatrics, I mean how many specialties can afford to say that among their patients there are book writers, right, are there few?
Since my patients are older than the common age, evidently they have achieved more goals than younger ones, I have met doctors in different sciences, masters, philosophus doctors, ex-military, people who worked clandestinely for many years, since they lived in difficult times in Venezuela as during the military dictatorship, people who survived natural disasters such as earthquakes or floods where only a few survived, people who arrived as first immigrants to certain cities, the first ones to perform certain activities, etc.
Por eso y muchos factores más, es que esto que les cuento termina convirtiéndose en una de las principales razones por las que escogí ser Geriatra, porque son estos adultos mayores las personas que tienen los mayores aprendizajes del mundo, es decir, los que te da la vida.
No me refiero solo a los aprendizajes producto de sus doctorados ni sus especialidades, es que, si hablas con un adulto mayor de forma consciente, te darás cuenta de que te darán los mejores consejos y te contarán las mejores historias, pero debes hacerlo con detenimiento ¡créeme!, Si lo haces así, vas apreciar todo eso que hay detrás de esos ojos opacos por su cristalino envejecido.
A lo largo de mi ejercicio me he dado cuenta de que pocos se sientan realmente a escuchar a un adulto mayor y esto en realidad es algo bastante triste, tomando en cuenta que muchos son estos seres que incluso son capaces de escribir libros de historia por sí solos, por eso que les digo arriba, es que he comprobado que la mayor medicina para muchos de ellos es que los escuchen.
En mis consultas en el hospital me he ganado a veces el desprecio de una parte del personal que prefiere ir y hacer un trabajo rápido para movilizar la mayor cantidad de pacientes posible y yo siempre he advertido que no puedo actuar por igual porque lo principal que quiere un adulto mayor es que lo escuches, ellos a lo que más le dan valor es al tiempo que le dedicas, eso es el mayor halago y la mejor de todas las terapias para sus enfermedades.
Because of this and many other factors, is that what I am telling you ends up becoming one of the main reasons why I chose to become a Geriatrician, because these older adults are the people who have the greatest learning in the world, that is, the ones that life gives you.
I am not only referring to the learnings from their doctorates or their specialties, but if you talk to an older adult consciously, you will realize that they will give you the best advice and tell you the best stories, but you must do it carefully, believe me, if you do so, you will appreciate all that is behind those eyes that are opaque because of their aged crystalline lens.
Throughout my practice I have realized that few really sit down to listen to an older adult and this is actually quite sad, taking into account that many are these beings who are even capable of writing history books by themselves, that is why I tell you above, is that I have found that the greatest medicine for many of them is to listen to them.
In my consultations at the hospital I have sometimes earned the contempt of some of the staff who prefer to go and do a quick job to mobilize as many patients as possible and I have always warned that I can not act equally because the main thing an elderly person wants is to be listened to, what they value most is the time you dedicate to them, that is the greatest compliment and the best of all therapies for their illnesses.
Estos 2 pacientes, que son escritores, me regalaron estos libros esta semana y son personas a los que aprecio mucho, porque sí, a mis pacientes yo les tomo un gran aprecio, con el tiempo me convierto hasta en su confidente, existen muchas cosas que piensan y no quieren contarles a sus familiares, así que muchas veces prefieren contármelas a mí primero.
Muchos, incluso, pareciera que solo me llaman porque quieren hablar conmigo y contarme lo que ha pasado últimamente en sus vidas, por eso siempre recalco que mis consultas son tardías porque esto forma parte fundamental de mi trabajo, el escuchar, además mientras escucho evalúo todas las esferas que forman parte del paciente, incluyendo la cognitiva y la emocional.
Muchas personas creen que cualquiera puede escuchar y, no es así, de hecho, es más difícil encontrar un buen “escuchador” que un buen “hablador”, porque, sí, muchos pueden hablar y decir cualquier tontería en el momento menos esperado, pero el que escucha y te hace comentarios elocuentes en el momento correcto, ese sabemos que en realidad te está prestando atención.
Por eso mis pacientes siempre me agradecen que los escuche y siempre lo hago con profunda admiración, porque lo hago con un pensamiento: “¿Cuántos conocimientos puede albergar esta persona? incluso más que los que posee cualquier biblioteca en el mundo”, por eso lo hago con mucho placer, siempre pensando que estoy aprendiendo algo de alguien valioso, mientras estoy ayudándolo a mejorar física y emocionalmente.
These two patients, who are writers, gave me these books this week and they are people I appreciate very much, because yes, I appreciate my patients very much, with time I even become their confidant, there are many things they think and do not want to tell their relatives, so many times they prefer to tell me first.
Many, even, it seems that they only call me because they want to talk to me and tell me what has happened lately in their lives, so I always emphasize that my consultations are late because this is a fundamental part of my work, listening, also while listening I evaluate all spheres that are part of the patient, including cognitive and emotional.
Many people believe that anyone can listen and, it is not so, in fact, it is more difficult to find a good "listener" than a good "talker", because, yes, many can talk and say any nonsense at the least expected moment, but the one who listens and gives you eloquent comments at the right time, that one we know is really paying attention to you.
That is why my patients always thank me for listening to them and I always do it with deep admiration, because I do it with a thought: "How much knowledge can this person hold? even more than any library in the world", that is why I do it with great pleasure, always thinking that I am learning something from someone valuable, while I am helping them to improve physically and emotionally.
En especial estos pacientes que me regalaron estos libros, tienen aproximadamente 80 años de vida, es decir, son 80 años de aprendizajes basados en la experiencia, ¿Díganme ustedes dónde podrán conseguir algo así? ¿Verdad que es invaluable?
Por todo esto, es que siempre digo que la experiencia lo es todo amigos, de nada nos sirve aprender cosas cuando no las ponemos en práctica y esa práctica es la que nos dará experiencia, experiencia que se adquiere con el pasar de los años y haciéndonos mayores como esos pacientes que yo atiendo.
