I recently went camping near the Lava Beds National Monument in California. This stairway leads down to an underground lava cave.
Some of the caves here are over a mile long, and ceiling heights can be as low as 1ft high! My companion and I often had to crawl and sometimes slide on our bellies to get to the next section of cave.
