Hoy recibí una llamada de un paciente que refiere haber sufrido un traumatismo que le causa dolor al masticar, sensibilidad al frío, por lo tanto le es doloroso al comer y tomar bebidas frías, y como nos encontramos en aislamiento, le agendamos una hora para poder abrir la clínica y atenderlo sólo a él.

Today I received a call from a patient who refers to having suffered a trauma that causes pain when chewing, sensitivity to cold, therefore it is painful to eat and drink cold drinks, and as we are in isolation, we scheduled an hour to open the clinic and attend only to him.

Al examen clínico, se observa fractura de la cúspide vestibular del 15 (primer premolar superior derecho), dejando expuesta la dentina, por eso la sensibilidad, debido al dolor se coloca anestesia infiltrativa, para así poder darle retención y aliviar los bordes de la fractura, ya que hay que extender para que el material se adhiera mejor.

On clinical examination, a fracture of the vestibular cusp of 15 (first upper right premolar) is observed, leaving the dentine exposed, so sensitivity, due to pain, is placed under infiltrative anesthesia, in order to give retention and relieve the edges of the fracture, since it is necessary to extend so that the material adheres better.

Se realiza la desmineralización con ácido fosfórico al 32%, luego se coloca el adhesivo, se realiza la reconstrucción con resina foto curada color A2 de ivoclare, se talla se hace el chequeo de oclusión y se pule.

The demineralization is done with 32% phosphoric acid, then the adhesive is placed, the reconstruction is done with ivoclare A2 color photo cured resin, the occlusion check is done and it is polished.

Se le recomienda al paciente no comer alimentos muy duros o tostados ya que no es lo mismo la resina que el esmalte dental y a pesar de que la resina es resistente a la abrasión a la tensión superficial, al desgaste y a los choques masticatorios jamas será tan resistente que el propio esmalte.

It is recommended that the patient does not eat very hard or roasted food as resin is not the same as tooth enamel and although the resin is resistant to abrasion, surface tension, wear and masticatory shocks it will never be as resistant as the enamel itself.
