Today evening one Pakistani middle-aged patients came with his friends to my chamber. He had an interesting and dangerous disease history the previous night. He was a truck driver of a company which is given the task of waste management by the municipality. The previous night, when they were collecting garbages from the dustbins around the city, suddenly he became unconscious at the driving seat. He was driving the truck. There was a helper beside him. According to the helper, there was a sudden break of the vehicle followed by some convulsive movement of the body of the patient. As the car was stopped, they brought him out. He was unconscious. There was froth coming out of his mouth. Also, there was a tongue bite. According to the witness, he was unconscious for around one hour. After coming out of the unconsciousness, he vomited once. He also felt a severe headache. These all took place at 2 am. According to the patient, he had no previous similar history.
He came to me today evening (around 15 hours after the incidence). He was a little shaken but he was talking freely and he came walking by himself. I found all of his vitals are within normal limit except bite marks on his tongue. He was still having a headache. He wasn’t having any one-sided weakness. I checked his blood sugar and did his ECG. Both of the tests were normal.
There was no obvious diagnosis (yet to me). My provisional diagnosis was either “a case of seizure/convulsion” or “a case of TIA (transient ischemic attack)”. I gave some treatment for his headache and gave one referral letter to go to a higher centre for a detailed check-up.
My today's #win.
