There are three instances I can recall.

The lowest of the three is when I was... fourteen? Somewhere around there, maybe younger. Cut my arm open on Barb Wire. That isn't' what hurt so bad, even though the numbing wore off while they were stitching it multiple times (It was a big cut). It came a few weaks later when they realized there was an infection because it wasn't properly cleaned, and the doctor lightly squeezed my wrist and I burst into tears. (No, the cut wasn't on my wrist, the infection travled up my arm).

THe most painful is between two moments. When my wisdom teeth broke through they did it long after I should have gotten them removed, just never got around to it. They couldn't be properly cleaned because they never came out, and they had already started rotting by that point. So the moment they were exposed to air it was a very long stretch of pain before I got them pulled.

The next moment was worse for that instant, and by a fair margin, but only lasted a moment before the pain subsided. And that is when I got my face waxed. I have really, really thick hairs on my face, and the guy who waxed me was not weak by any measure. First time He pulled he could not get the wax on my upper lip off. When he finally did there was an instant rush of blood and tears. Mind you the skin on my face didn't rip at all, it's just how thick my hairs were, healed up just fine. But holy shit that hurt.
