Cardiac tamponade is a life-threatening condition in which fluid (blood, pus, serous fluid, etc.)
Fluid accumulates in the pericardial sac.
The pericardium is a fluid-filled, double walled fibro-serous sac that encloses the heart and the roots of the major blood vessels
• Trauma: Penetrating or blunt chest trauma • Iatrogenic/Procedural: Complications from invasive cardiac procedures • Inflammatory and Infectious: Acute pericarditis caused by viral, bacterial, fungal, or tuberculous infections. • Malignancy: Primary cardiac tumors or, far more commonly, metastatic cancers • Metabolic: Severe hypothyroidism (myxedema) or uremia secondary to kidney failure. • Cardiovascular Pathology: Myocardial infarction leading to free wall rupture, or an aortic dissection leaking into the pericardial sac.
Beck's Triad
The classic clinical signs of acute cardiac tamponade are collectively known as Beck's Triad:
Hypotension (with a narrowed pulse pressure) due to decreased stroke volume.
Jugular Venous Distension (JVD) reflecting elevated systemic venous pressure.
Muffled or Distant Heart Sounds caused by the insulating effect of the surrounding pericardial fluid.
Other Key Signs and Symptoms
• Pulsus Paradoxus
• Dyspnea and Tachypnea
• Tachycardia:
• Signs of Obstructive Shock: Cool, clammy skin, altered mental status, and oliguria (decreased urine output).
• Ewart's Sign

Diagnosis
Chamber Collapse: The right atrium and right ventricle physically collapse under pressure.
Swinging Heart: The heart rocks back and forth inside the massive pool of fluid.
Dilated IVC: The inferior vena cava stays swollen and fails to collapse when the patient breathes.
Sinus Tachycardia: A fast heart rate as the body fights to keep blood moving.
Low QRS Voltage: Small electrical waves because the surrounding fluid dampens the electrical signal.
Electrical Alternans: The heights of the ECG waves alternate from beat to beat because the heart is swinging.

• Supplemental oxygen to reduce work loac the heart.
• IV fluids to maintain normal BP
• Antibiotics
•Inotropic drugs if patient is severely hypotensive drugs eg Dopamine, Dorbutamine.
• Avoid diuretics or vasodilators
• Bed rest with leg elevation
