I am having a difficult time understanding how a problem like that creeps up on you.

At a basic level is what happened is we got to much rain for the dam to hold back the water, they would release some of the water (so the dam does not bust) but they can not do that because someone forgot to maintain the relief spill way?

Is that basically the story?
