Penile implants are devices placed inside the penis to allow men with Erectile Dysfunction (ED) to have an erection. Penile implants are usually recommended after other treatments for erectile dysfunction fail.
 

There are two main types of penile implants, Semi-Rigid Penile Prosthesis,and inflatable. Each type of penile implant works differently and has several advantages and disadvantages.
 

The placement of penile implants requires surgery. Before choosing penile implants, be sure to understand what the surgery involves, including possible risks, complications, and follow-up care.
 

For most men, erectile dysfunction can be treated successfully with medications or with the use of a penis pump (vacuum constriction device). You may consider penile implants if you are not a candidate for other treatments or if you cannot get a sufficient erection for sexual activity using other methods.
 

Penile implants can also be used to treat severe cases of a condition that causes scarring inside the penis, resulting in painful and curved erections (Peyronie’s disease).
 

Penile implants are not for everyone. Your doctor may be cautious about penile implants if you have:
 

ED which is situational, the result of a relationship conflict or potentially reversible
An infection, such as a lung infection or a urinary tract infection. Dermatitis, wounds, or skin lesions in your penis or scrotum keep in mind that while penile implants allow men to have an erection, they do not increase sexual sensation or desire. Most penile implants will also not make your penis larger than it is naturally at the time of surgery. In fact, his erect penis may be a little shorter than it used to be. 

The risks of penile implant surgery include:
 

Infection. As with any surgery, infection is a possibility. You may have an increased risk of infection if you have a spinal cord injury or diabetes. Men who need surgery to adjust or replace an implant (revision surgery) have an increased risk of infection than the first surgery.
 

Implant problems. New designs of penile implants are reliable, but in rare cases, implants may not work properly. For example, on some Semi-Rigid Penile Prosthesis, the internal parts can break down over time. On the inflatable devices, the liquid can drip or the pump device can fail. Surgery is necessary to remove, repair, or replace a broken implant.
 

Internal erosion or adherence. In some cases, an implant may adhere to the skin inside the penis or wear the skin from inside the penis. On rare occasions, an implant breaks through the skin. These problems are sometimes related to an infection.
 

Treat an infection
 

An infection can occur at any time after surgery. In rare cases, infection occurs years later. A severe infection can cause swelling of the scrotum, accumulation of pus, and fever. Surgery to remove the implant is always necessary to treat an infection. Replacing a penis implant can be complicated and costly and can lead to the accumulation of scar tissue, loss of sensation in the penis and a decrease in penis length.
 

Initially, you will talk to your doctor or a urologist about penile implants. During your visit, your doctor will probably:
 

Check his medical records. Be prepared to answer questions about current and past medical conditions, especially your experience with. Talk about any medications you are taking or have taken recently, as well as any surgery you have had.
 

Do a physical exam. To make sure that penile implants are the best options for you, your doctor will perform a physical exam, including a complete urological exam. Your doctor will confirm the presence and nature of erectile dysfunction and ensure that your erectile dysfunction cannot be treated differently. He or she will also try to determine if there is any reason why penile implant surgery may cause complications. Your doctor will also examine your ability to use your hands, as some penile implants require more manual dexterity than others.
 

Discuss your expectations. Make sure you understand the procedure and the type of penile implant that suits you the most. Your doctor will also explain the benefits and risks, including possible complications. Ideally, you will include your partner in the discussion with your doctor.
 

Types of penile implants

There are two main types of penile implants: 

Inflatable implants. Inflatable devices, the most common type of penile implant used, can be inflated to create an erection and deflate at other times. Three-piece inflatable implants use a fluid-filled reservoir implanted under the abdominal wall, a pump and a release valve placed inside the scrotum, and two inflatable cylinders inside the penis. To achieve an erection, you pump the fluid from the tank to the cylinders. Then, release the valve into the scrotum to drain the fluid back into the reservoir. The two-piece model works in a similar way, but the liquid reservoir is part of the pump implanted in the scrotum.
 

 Semi-Rigid Penile Prosthesis are always firm. The penis can be folded away from the body for sexual activity and toward the body to hide it. A positionable penile implant is a rigid device with a central series of segments that are held together with a spring at each end. You can keep the positions up and down better than other Semi-Rigid Penile Prosthesis.
 

Other special designs can provide longer length or adapt to a shortened penis. Some inflatable penile implants are also available with antibiotic coatings, which may help reduce the risk of infection.
