El cigarrillo es uno de los vicios y excesos más comunes a nivel mundial pero tiende afectar progresivamente nuestra salud en general debido a las sustancias carcinogenicas que este presenta, particularmente les mostraré este caso clínico de mi paciente adulta que actualmente atiendo en la facultad de Odontología, es una paciente femenina de 72 años de edad sistémicamente comprometida ya que sufre de hipertensión y también de arritmias cardíacas, esta paciente particularmente lleva 50 años fumando, por lo que muy difícil podrá erradicar por completo este hábito del cigarrillo.
Smoking is one of the most common vices and excesses worldwide but it tends to progressively affect our health in general due to the carcinogenic substances it contains. I will particularly show you this clinical case of my adult patient that I currently treat at the Faculty of Dentistry, she is a 72 year old female patient who is systemically compromised as she suffers from hypertension and also cardiac arrhythmias, this particular patient has been smoking for 50 years, making it very difficult to completely eradicate this cigarette habit.
Las consecuencias del cigarrillo en la cavidad bucal son totalmente nocivas, ya que la boca esta constituida de diversas estructuras duras y blandas de la cavidad bucal, siendo los dientes lo mas afectados, en la paciente se puede observar que es una edentula parcial, es decir que tiene varias piezas dentarias ausentes.
The consequences of smoking in the oral cavity are totally harmful, as the mouth is made up of various hard and soft structures of the oral cavity, with the teeth being the most affected. The patient is partially edentulous, i.e. she has several missing teeth.
Entre todas las manifestaciones clínicas que se pueden presentar en boca, están las cavidades en los dientes, será totalmente fácil y rápido que las caries dental aparezca en personas que consumen tabaco, debido a que las sustancias carcinogenicas y la nicotina, originan una disminución de la capacidad de producir saliva por lo que permite rápidamente la acumulación de bacterias en todos los dientes y también en las encías.
Among all the clinical manifestations that can occur in the mouth, cavities in the teeth, it will be very easy and quick for dental caries to appear in people who use tobacco, due to the fact that carcinogenic substances and nicotine cause a decrease in the capacity to produce saliva, which quickly allows the accumulation of bacteria in all the teeth and also in the gums.
Como podemos observar los labios están totalmente resecos, debido al contacto estrecho que estos poseen con el cigarrillo, el calor produce resequedad en los labios así como también cambios en la coloración de los labios estos se van oscureciendo, la mucosa de los labios se afecta totalmente, en color y textura.
As we can see the lips are totally dry, due to the close contact they have with the cigarette, the heat produces dryness in the lips as well as changes in the colouring of the lips, these become darker, the mucous membrane of the lips is totally affected, in colour and texture.
En esta paciente se observa como el paladar duro no tiene su color característico normal, sino que lo podemos evidenciar de color grisáceo, esta es una principal característica que nos permite corroborar que el paciente es fumador ya que la nicotina es quien mancha las estructuras.
In this patient we can see how the hard palate does not have its normal characteristic colour, but rather it is greyish in colour, this is a main characteristic that allows us to corroborate that the patient is a smoker, as it is nicotine that stains the structures of the palate.
Ahora bien en las encías podemos observar las manchas melánicas, en algunos casos son totalmente normales en pacientes con piel morena ya que es un cambio de pigmentación en la piel, no resulta ser patológico es normal pero en este caso ya pase ser asociados al tabaco y pasa hacer melanosis del fumador, podemos encontrarlas en los tres tipos de encías, solo es una pigmentación focal benigna, cabe destacar que los melonocitos son las células encargas de producir melanina que se encarga de garantizar la pigmentación de la piel.
Now in the gums we can observe melanic spots, in some cases they are completely normal in patients with brown skin as it is a change of pigmentation in the skin, it is not pathological it is normal but in this case it is associated with tobacco and happens to be smoker's melanosis, we can find them in the three types of gums, it is only a benign focal pigmentation, it should be noted that the melonocytes are the cells responsible for producing melanin which is responsible for ensuring the pigmentation of the skin.
Fumar enferma y además es totalmente negativa para todo el organismo en general, siempre hay factor psicológico que juega en contra es por ello que para eliminar el hábito es necesario recurrir al psicólogo.
