Not everybody wants to be helped. Some people enjoy living like this, like it or not. It is familiar. And when it is familiar it is close. When it's close it's known and when something is unknow we as human beings fear it on subconscious level. Change is hard. Staying the same is easy. People chose easy 99% of the time, even if they can live better, happier and more fulfilling lives. Some people call this Karma. You can't help everyone, but bless your good heart for trying!
