Here is the information you will need to provide to verify your identity:

• Accurate identifying information
• A photo of your ID in PNG, JPG, or JPEG file format
• A clear photo of your face next to a sheet of paper with the words “Oath” and today’s date written on it in PNG, JPG, or JPEG file format
