That people have no access to health care. That people become poor and homeless while Republican supported fossil fuel companies get richer and richer destroying the environment (because Trump removed the USA from international agreements and gave the companies permission to take over protected natural reserves including native American lands.).

According to recent polls, the vast majority of Republicans believe that Qanon is true to some degree (some believe it completely, some most, some part). That says something about the intellectual makeup of these people. If Trump won, humanity will not stand a chance in fighting climate change as the world can't do it without the USA.
