All those three are Orthodox Christian countries and I found an interesting statement in Quora:

"You must declare yourself as a pious Greek Orthodox to be a Tzar. Kings enjoy much more wiggle room in terms of religious affiliation. Our Polish Tzar Władysław IV Vasa tried holding the throne as a Catholic, and ended up a joke."
