매독 (syphilis)

1.   원인(cause) : T. pallidum à , sexual contact, placenta infection. 성접촉, 태반감염으로 인체에 감염된다.
   

2.   역학(incidence) : 50%. weakly infectivity감염력이 약하다.
   

living together over 1year 224 couple , infectiontious rate 50%, so very weakly infectious dz.

à 1년 이상 동거한 224쌍의 부부를 대상으로 매독환자의 배우자 감염률을 살펴보면, 약 50% 만이 감염되었다. 이와 같이 매독균은 전염력이 매우 약함을 알 수 있다.

1.   Coarse of syphilis 매독의 임상기
   

1. aquired syphilis,  후천성 매독
   

① 1st period syphilis 1기 매독

② 2nd period syphilis 2기 매독

③ Incubation syphilis 잠복매독

early incubation syphils : under 1year after infection 조기잠복매독: 감염 후 1년 이내

lat incubation syphilis : over 1year after infection 후기잠복매독: 감염 후 1년 이상

④ 3rd period syphilis3기 매독

1.  apriority syiphilis 선천성 매독
   

① early apriority syphilis 조기 선천성 매독: under 2years at birth 생후 2년 이내에 나타나는 매독증세

② late apriority syphilis 후기 선천성 매독: over 2years at birth 생후 2년 이후에 나타나는 매독증세

1.   임상증상 clinical expression
   

1st, , 2nd, 3rd trimester syphilis and apriority syphilis seen variaty clinical manifestationn, especially skin, mucosa, internal organ. but generaly no clinical manifestation. and self resolving 30-40% 1기, 2기, 3기 매독과 선천성 매독에서 피부, 점막, 내부장기에 매우 다양한 병변이 발생한다. 일반적으로 매독의 병변에는 소양증이나 통증 등의 자각 증세가 없다. 매독의 30~40%는 자연치유

u 1st period , 1기 매독

1.  Chancre(경성하감)
   

2.  after infection 3wks , usuallly 1 ulcer create, no painful 감염 3주 후에 매독균이 침범한 부위에 보통 통증이 없는 1개의 궤양이 발생한다.
   

3.  «««통증이 없는 궤양à carilage hard: 딱딱
   

4.  mostly genital ulcer, and self resolving after 3-8wks later 주로 성기에 발생하며, 3~8주 후에 자연소실된다.
   

u 2nd period 2기 매독

1.  매독진(syphilid)
   

2. muculopapular syphilid 반점상 매독진 – first finding 가장 먼저 나타남, 경계가 불명확한 홍색 반점이 전신, 손바박 발바박에 나타난다.
   

3. papular syphilid 구진상 매독진 – 구진인설상 매독진은 장미색비강진과 감별
   

4. generally 3wks-12wks expression after 1st period 1기 매독증세가 발생한 후 3~12주에 나타난다.

5. syphilis symptom expreesion (variety symptom) on skin and mucosa and hand and foot 피부와 점막에 다양한 매독 증세가 나타나고, 손바닥, 발바닥에 흔히 발생한다. à 반점양 매독진
   

6.  몸에는 구진양 매독진
   

7. ««머리는 Moth-eaten alopecia(매독성 탈모증)
   

8. If syphilis is in progress blood infection, can go any organ. but profer treatment at 2nd period, easily cure 혈행을 통해 어느 내부장기에도 매독균이 침범할 수 있으나 2기 매독을 적절히 치료하면
   

신경매독으로 발전하지는 않는다.

u Incubation syhphilis : no clinical manifestation, CSF negative test, but serum test positive 잠복매독: 증세 없이 매독 혈청검사에서 양성을 보이고, CSF 검사에서 이상 소견이 없는 매독

u 3rd period 3기 매독

1.  양성 3기 매독
   

2.  신경매독 neurosyphilis
   

3.  심장혈관매독 coronary a. syphilis
   

1.   매독의 경과 syphilis progress
   

1.  30%는 자연치유 30% self cure
   

2.  60~70%는 잠복매독으로 평생경과, 3기 매독으로 발전 60-70% incubation syphilis for all life, progressed 3rd period
