For new CastGarden channels, we need: 
1. Desired peertube username (the content owner.name (to edit your content)): 
2. Channel name (your brand (podcast name)) for the url, ie: cast.garden/c/channelname: 
NOTE: Peertube username and Channel name cannot match. Use only alphanumeric characters, dots, underscores.
3. Web3 "Hive" username (so you can login, get paid, reply and comment, get stats, etc): 
4. URL to your existing podcast (if possible): 
5. Do any Hive streaming sites already monetize your podcast? Yes/No: 
6. 192x192 channel avatar image:

7. Linkback Proof (syndication begins after you post your CastGarden channel link somewhere):
