This is the first time I've seen "dystrophic processes" related to gout. But I don't understand it. Do you have any links to explanations?
For pain control, you're right to suggest NSAIDs. Indeed, I took ibuprofen for many years before I learned that long term usage is a risk for heart disease. So I started allopurinol to control the excess uric acid that causes gout.
Allopurinol takes several months to get uric acid completely under control. During which time, there's still a liklihood of painful flares. Colchicine is often effective. But it's important to limit the dosage in line with modern recommendations.
