I wanted to inform you of the following:

a) COBRA ends 11-30-2010

b) I have not received their October or November checks in the amount of 154.00

If I do not receive by November 15th I am going to terminate coverage.

Please have them Fed Ex or wire funds as I should not have to act like a collection agency
