All of the above are incredibly annoying, and particularly people who are oblivious of their surroundings.

I'll add to that people in cars who behave like they clearly are the only people to ever use the road... swerving as they text, slowing down to crawl because they see an interesting shop in the middle of heavy traffic, then going too fast through a school zone and finally making a sudden left turn from the right hand of 4 lanes of traffic...
