How does one start an EOS blockchain? Who decides which EOS blockchains are valid and can interoperate? I have been reading the white paper and would like more information about the practical implementation

If I start an EOS blockchain do I just create my own tokens on it?
I am trying to understand the relation between a new EOS blockchain, the EOS ICO tokens, and tokens on a new blockchain. Do I just issue my own tokens on my chain, or are they derived from the ICO tokens?
