Even thought the Joker is dead he made his place in the one place he could never have gotten alive, inside Batman’s head. Here, Joker declares his intention of making Batman kill again. Again as in Batman’s consciousness he thinks that he killed the Joker. Batman wouldn’t have hesitated to give the cure to the Joker has the Joker not stabbed him in his hand during their fight which made Bruce drop the cure.
