El d铆a promet铆a retos y glorias. Embarcaciones, c谩maras en mano y gente, compartieron unos d铆as, como un cardumen de peces en las aguas cristalinas de Kalamata, Grecia.
La apne铆sta iniciar铆a en unos segundos un descenso a las profundidades del reino de Neptuno para intentar batir un r茅cord. 驴Le permitir谩n sus o铆dos bucear sin adversidades?
No es ficci贸n, amigos, es lo que se vivi贸 hace unos d铆as en el 8潞 Campeonato del Mundo de Apnea Profunda. Organizado por la Confederaci贸n Mundial de Actividades Subacu谩ticas (CMAS), del 2 al 13 de este mes, con la presencia de 37 pa铆ses.
Isabel estaba en buena preparaci贸n f铆sica y psicol贸gica, pero la realidad es que su actividad deportiva se desarrolla en las profundidades de un medio l铆quido, donde nadie escapa a la influencia de una de las variables f铆sicas m谩s importantes: la presi贸n.
Esto no solo se limita al buceo, en un avi贸n, al subir una monta帽a, en un ascensor o en otras situaciones que condicionan cambios de presi贸n; siempre sentimos en nuestros o铆dos los cambios de la presi贸n ambiental, como un centinela que nos avisa de la presencia de un intruso.
A nivel popular, las opiniones a veces son err贸neas. Muchos piensan que cuando estamos escalando o cuando estamos en un avi贸n, la presi贸n aumenta. Asocian el aumento de la presi贸n con la altitud a la que uno se encuentra.
En realidad, ocurre lo contrario: por encima del nivel del mar, la presi贸n disminuye. Por debajo del nivel del mar, aumenta.
Estos cambios de presi贸n en nuestro cuerpo se sienten, y recibimos las se帽ales de su presencia. Si estas diferencias de presi贸n no se equilibran, sufriremos s铆ntomas desagradables y da帽os en nuestra integridad. Esto solo se logra con una correcta maniobra de compensaci贸n.
Son t茅cnicas para equilibrar la presi贸n entre varios compartimentos que contienen espacios a茅reos, ejemplo: m谩scara de buceo, senos paranasales, piezas dentales, o铆dos, pulmones e intestinos.
Si no se logra el equilibrio de presiones, sufriremos lo que se conoce en medicina, como un Barotraumatismo.
Hoy nos centraremos en el o铆do. Este tiene 3 divisiones: o铆do externo, medio e interno. El o铆do medio es un espacio a茅reo y es un lugar diana que debe estar compensado, para evitar accidentes al buceador. Problemas en cada secci贸n de esta divisi贸n anat贸mica, tendr谩n influencia en las otras.
En el caso del o铆do medio, la efectividad de las maniobras de compensaci贸n, est谩 en relaci贸n con la permeabilidad e integridad de la trompa de Eustaquio.
El aire de las v铆as a茅reas superiores se comunica con la cavidad del o铆do medio a trav茅s de estas trompas, un conducto por cada o铆do, encargados de compensar los cambios de presi贸n entre ambas cavidades.
Esto es posible, si est谩 permeable y no tiene ninguna causa que obstruya el libre flujo del aire, como cuadros infecciosos, tumorales y anat贸micos.
En el ascenso disminuye la presi贸n ambiental, hay apertura de la trompa de Eustaquio, y el exceso de aire en el o铆do medio se desplaza pasivamente a trav茅s de ella. Casi siempre no se precisa realizar ninguna t茅cnica de compensaci贸n.
Si existiera una obstrucci贸n de la trompa, el o铆do medio tendr谩 una presi贸n positiva con respecto a la presi贸n en el exterior, que va disminuyendo con la emersi贸n del buzo. Este podr谩 sufrir un barotrauma timp谩nico explosivo (rotura de la membrana timp谩nica hacia el conducto auditivo externo).
En el descenso aumenta la presi贸n ambiental, con una disminuci贸n del volumen de aire en el o铆do medio. Se hace necesario recurrir a las maniobras de compensaci贸n para poder abrir las trompas y equilibrar las presiones.
Si no se logra, hay deformaci贸n de la membrana timp谩nica hacia adentro. Con sensaci贸n de taponamiento y dolor, que de no resolverse, llevar铆a a una lesi贸n de la membrana, conocido como barotrauma timp谩nico implosivo.
Este grado de lesi贸n se detecta al realizar una Otoscopia por parte de un especialista. El definir谩 el grado de lesi贸n, que va desde el enrojecimiento de la membrana hasta la perforaci贸n, como pueden ver en la foto de abajo.
A veces, con una trompa de Eustaquio normofuncional, pueden producirse barotraumatismos, si el conducto auditivo externo tiene una obstrucci贸n, por ejemplo, tapones de goma o de cerumen.
La m谩s usada es la maniobra de Valsalva. Consiste en pinzarnos la nariz con los dedos, mientras tratamos de expulsar el aire por ella, con la boca cerrada. Sentiremos un peque帽o sonido en ambos o铆dos, si se han equilibrado las presiones correctamente.
Otras m谩s fisiol贸gicas, son la maniobra de Frenzel y la maniobra de Toynbee. El que guste profundizar en el tema, puede hacerlo en mis referencias finales.
Durante esta competencia en Grecia, Isabel present贸 un percance en su o铆do derecho, pero gracias a su experiencia y su ecuanimidad, supo lidiar con esa limitante; regalando a los presentes, una de sus mejores actuaciones.
No imagin贸 que su primera presea llegar铆a en la modalidad de Peso Constante sin Aletas. Llegando a los 57 metros de profundidad. para posicionarse con una nueva marca nacional, con medalla de bronce; a pesar de que su plato fuerte, es el uso de la bialeta y la inmersi贸n libre. Modalidad donde descendi贸 hasta 86 metros, logrando un r茅cord nacional, y quinta posici贸n a nivel mundial.
La apne铆sta logr贸 pintar en ese campeonato de Kalamata, dos r茅cords de Espa帽a y una medalla de bronce mundial.
Isabel S谩nchez-Ar谩n se enfrent贸 al titular de este post; pero supo sobreponerse, mostrando una respuesta positiva, reflejo de su concentraci贸n, disciplina y pasi贸n por la apnea. Fue capaz de superar un obst谩culo en su cuerpo y lograr la victoria.

The day promised challenges and glories. Boats, cameras in hand and people, shared a few days, like a school of fish in the crystal clear waters of Kalamata, Greece.
The freediver would begin in a few seconds a descent into the depths of Neptune's kingdom to try to break a record. Will her ears allow her to dive without adversity?
This is not fiction, friends, this is what was experienced a few days ago at the 8th World Championship of Deep Freediving. Organized by the World Confederation of Underwater Activities (CMAS), from the 2nd to the 13th of this month, with the presence of 37 countries.
Isabel was in good physical and psychological preparation, but the reality is that her sporting activity takes place in the depths of a liquid medium, where no one escapes the influence of one of the most important physical variables: pressure.
This is not only limited to diving, in an airplane, when climbing a mountain, in an elevator or in other situations that condition pressure changes; we always feel in our ears the changes in ambient pressure, like that sentinel that warns us of the presence of an intruder.
At the popular level, opinions are sometimes mistaken. Many people think that when we are climbing or when we are in an airplane, the pressure increases. They associate the increase in pressure with the altitude at which one is.
In reality, the opposite is true: above sea level, pressure decreases. Below sea level, it increases.
These pressure changes in our body are felt, and we receive signals of their presence. If these pressure differences are not balanced, we will suffer unpleasant symptoms and damage to our integrity. This can only be achieved with a correct compensation maneuver.
They are techniques to balance the pressure between various compartments containing air spaces, for example: diving mask, sinuses, teeth, ears, lungs and intestines.
If the pressure balance is not achieved, we will suffer what is known in medicine as a Barotrauma.
Today we will focus on the ear, which has 3 divisions: outer, middle and inner ear. The middle ear is an air space and is a target site that must be compensated to avoid accidents to the diver. Problems in each section of this anatomical division will influence the others.
In the case of the middle ear, the effectiveness of compensation maneuvers is related to the patency and integrity of the Eustachian tube.
The air from the upper airways communicates with the middle ear cavity through these tubes, one duct for each ear, which are responsible for compensating the pressure changes between the two cavities.
This is possible, if it is permeable and does not have any cause that obstructs the free flow of air, such as infectious, tumor and anatomical conditions.
During ascent, the ambient pressure decreases, the Eustachian tube opens, and the excess air in the middle ear is passively displaced through the tube. Almost always there is no need to perform any compensation technique.
If there is an obstruction of the tube, the middle ear will have a positive pressure with respect to the pressure outside, which decreases with the diver's emersion. The diver may suffer an explosive tympanic barotrauma (rupture of the tympanic membrane towards the external auditory canal).
During the descent, the ambient pressure increases, with a decrease in the volume of air in the middle ear, leading to closure of the nasopharyngeal portion of the tube.
It becomes necessary to resort to compensatory maneuvers to open the tubes and balance the pressures.
If this is not achieved, an inward deformation of the tympanic membrane occurs. With a sensation of tamponade and pain, which if not resolved, would result in a lesion known as implosive tympanic barotrauma.
This degree of injury is detected by performing an Otoscopy by a specialist. He will define the degree of injury, ranging from redness of the membrane to perforation, as you can see in the picture below.
Sometimes, with a normally functioning Eustachian tube, barotrauma can occur if the external auditory canal has an obstruction, e.g. rubber or earwax plugs.
The most used is the Valsalva maneuver. It consists of pinching our nose with our fingers, while we try to expel air through it, with our mouth closed. We will feel a small sound in both ears, if the pressures have been balanced correctly.
Other more physiological maneuvers are the Frenzel maneuver and the Toynbee maneuver. If you would like to go deeper into the subject, you can do so in my final references.
During this competition in Greece, Isabel presented a mishap in her right ear, but thanks to her experience and her equanimity, she knew how to deal with this limitation; giving to those present, one of her best performances.
She did not imagine that her first medal would come in the modality of Constant Weight without Fins. Reaching a depth of 57 meters, to position herself with a new national record, with a bronze medal; although her highlight is the use of the bi-fins and free immersion. Modality where she descended to 86 meters, achieving a national record, and fifth position in the world.
The freediver managed to paint in this championship of Kalamata, two records of Spain and a world bronze medal.
Isabel S谩nchez-Ar谩n faced the headline of this post; but she was able to overcome, showing a positive response, reflecting her concentration, discipline and passion for freediving. She was able to overcome an obstacle in her body and achieve victory.
