IX Spearfishing Championship in Chile: medical considerations

Hello friends of Full Deportes, the IX Pan American Spearfishing Championship has just finished in Chile. Sporting event that was organized by ADESUBA sponsored by CMAS America and FEDESUB, from 10 to 13 of this month, and held in the waters of the port city of Antofagasta.

Teams from several countries participated, such as Peru, Brazil, Uruguay and Argentina.
In the results, Chile took first place with fisherman Esteban Sierra, and third place with Miguel Angel Soto. The second place went to Peru, with the competitor Sergio Espinoza.

The introduction of this post, leads me to share with you important medical considerations, to which these athletes undergo in their fishing activity. This way we add more usefulness to the writing, than just putting names and places of a sporting culmination.

Spearfishing is an action that takes place in an aquatic environment. It is a diving in apnea, immersion, where breathing is voluntarily suspended.
It is most often done in the sea, but there are fishermen who do it in rivers and lakes.

At a sporting level, athletes must undergo constant training that allows them to be in good health conditions, both physically and psychologically.
There must be harmony between both aspects. One factor cannot be in predominance or deficit with respect to the other.

Sometimes, there are health problems or risk factors that the person is unaware of. Hence the importance of undergoing periodic health examinations.

Ideally, these examinations should be performed in an active hospital service of Underwater and Hyperbaric Medicine, where other clinical-surgical specialties are integrated, in order to detect certain risk conditions or health problems that are a relative or absolute contraindication for the safest development of the activity.

These athletes also undergo barofunction and oxygen tolerance tests inside a hyperbaric chamber.

On a physical level, we must rule out cardiovascular problems, such as arrhythmias, severe valvular lesions or problems that may lead to myocardial insufficiency and ischemia.

Diving causes an increase in cardiac preload, which, added to peripheral vasoconstriction, leads to elevated blood pressure.

The integrity of the otorhinolaryngological system is one of the causes of many diving accidents. Adequate compensation in the external and middle ear and paranasal sinuses is of great importance to avoid barotraumatic pathologies during the diver's ascent and descent.

At the respiratory system level, pulmonary hypertension, spontaneous pneumothorax, and other problems that cause respiratory deficiencies should be ruled out.

In the central nervous system, any condition that may lead to loss of consciousness may lead to drowning and death.

The digestive, endocrine, ostemioarticular and hematological systems are also evaluated by each of the specialists that make up the medical commission for the periodic evaluation of the divers.

The other aspect to be evaluated is the psychological harmony of the person. This is of great significance for the full activity of diving, and sometimes it is difficult to be detected.

A diver may have all the systems described above working perfectly, but if a mental imbalance occurs during a dive, he may react with panic and not take the preventive measures and actions he normally knew.

Every sport federation should promote, know and be updated, in a body of theoretical knowledge, where the physical laws of gases, some physiology and underwater medicine, first aid, and everything related to prevention and initial measures, before any type of diving accident that occurs.

I hope that the championship in Chile, has ended without any setback from the medical point of view.
