Mrs. Diaz, 61 years old, born in the state of Aragua but living in the city of San Juan de los Morros, has a pathological history of cystic plexiform ameloblastoma diagnosed in 2012, and at the end of that same year she was taken to the operating table and underwent total exeresis (extraction) of the tumor that had invaded her left maxillary sinus and thus destroyed the anterior wall of the same.
La Sra. Díaz de 61 años de edad, nacida en el estado Aragua pero vive en la ciudad de San Juan de los Morros, tiene como antecedente patológico ameloblastoma quístico plexiforme diagnosticado en el año 2012, y a finales de ese mismo año la llevan a mesa operatoria y le hacen excéresis (extracción) total del tumor que le había invadido seno maxilar izquierdo y por ende le destruyó la pared anterior del mismo.
By the year 2015, in her annual analysis there was no evidence of regeneration of the lesion, apparently everything was going very well. Sadly, in the 2018 check-up, a reappearance of the tumor was again evidenced, in the same site, left maxillary sinus; immediately outpatient treatment with steroids and anti-inflammatory drugs was started, but without any result.
Para el año 2015, en sus análisis anuales no se evidenciaba regeneración de la lesión, al parecer todo marchaba muy bien. Tristemente, en el chequeo del 2018 se evidencia nuevamente una reaparición del tumor, en el mismo sitio, seno maxilar izquierdo; de inmediato se inició tratamiento ambulatorio a base de esteroides y antiinflamatorios, pero sin ningún resultado.
Due to persistent symptomatology and the addition of blood discharge from the nostrils to the clinical picture, she decided to see a maxillofacial specialist who evaluated the patient and referred her to the otorhinolaryngology service. Thus, the patient came to our office that same year, for which she received outpatient and antihemorrhagic treatments.
Por persistir sintomatología y agregarse al cuadro clínico salida de secreción hemática por fosas nasales, decide acudir a especialista maxilofacial quién valora indica laboratorios y refiere al servicio de otorrinolaringología. Es así, como la paciente llega a nuestra consulta ese mismo año, para lo cual recibió tratamientos ambulatorios y antihemorrágicos.
In this sense, years passed and Mrs. Diaz bled monthly but no doctor solved it. On July 3rd of this year, she began to bleed at home at 10:00 p.m. from the left nostril, they tried to stop the bleeding, but it continued, so she decided to go to the hospital. It was an honor to be on duty and receive her!
En este sentido, pasan los años y la Sra. Díaz sangraba mensualmente pero ningún médico la resuelve. El pasado 03 de julio del presente año, comienza a sangrar en casa a las 10:00 p.m. por la fosa nasal izquierda, intentaron pararle la hemorragia, pero esta continuaba, por lo que decide acudir al hospital. ¡Fue un honor estar de guardia y recibirla!
The first days were not easy, every time the wick had to be changed, the amount of blood that came out was impressive, but it managed to stop. Days went by and it was agreed with the maxillofacial specialist and otolaryngologist that in a period of no less than 15 days the patient would be taken to the operating table again to remove the tumor, since it had already invaded the maxillary sinus, ethmoidal sinus, frontal sinus, hard palate, left mandible and left nostril.
Los primeros días no fueron fácil, cada vez que tocaba cambio de mecha, la cantidad de sangre que salía era impresionante, pero se lograba parar. Pasaron los días y se pudo acordar con el especialista de maxilofacial y otorrinolaringólogo que en un lapso no menor a 15 días la paciente se llevará a mesa operatoria nuevamente para extraer el tumor, puesto que ya ha invadido seno maxilar, seno etmoidal, seno frontal, paladar duro, mandíbula izquierda y fosa nasal izquierda.
You may ask, how can this tumor invade so many anatomical cavities and still not be malignant? Well, Ameloblastoma is usually asymptomatic in its early stages and is often seen as a finding during a routine radiographic examination. It usually occurs in the mandible and is diagnosed mainly between the fourth and fifth decade of life, with no gender predilection.
Ustedes se preguntarán, ¿Cómo este tumor puede invadir tantas cavidades anatómicas y aun así no es maligno? Pues, el Ameloblastoma es normalmente asintomático en sus fases iniciales y a menudo se observa como hallazgo durante una exploración radiográfica de rutina. Por lo general se presenta en la mandíbula, se diagnostica sobre todo entre la cuarta y quinta década de vida, sin existir predilección por género.
It is a benign tumor with a high rate of recurrence, being considered by R. Marx as one of the four most aggressive lesions due to its clinical and histological characteristics, so it must be treated with resection and safety margins, which is why radical surgical treatment is the first choice.
Es un tumor benigno con un alto índice de recidivas, siendo considerado por R. Marx como una de las cuatro lesiones más agresivas por sus características clínicas e histológicas, por lo que debe ser tratado con resección y márgenes de seguridad, razón por la cual el tratamiento quirúrgico radical es la primera elección.
Plexiform ameloblastoma in its unicystic variant presents ameloblastic epithelium in the context of the cystic wall. The clinical case presented an ameloblastoma with the histopathological subtype of intramural growth, Ackerman's subtype IIIa, which invaded the capsule and the underlying bone tissue. According to the literature, the appropriate treatment for this type of lesion is radical treatment.
El ameloblastoma plexiforme en su variante uniquística presenta epitelio ameloblástico en el contexto de la pared quística. El caso clínico expuesto presenta un ameloblastoma con el subtipo histopatológico de crecimiento intramural, subtipo IIIa de Ackerman que invadía la cápsula y el tejido óseo subyacente. De acuerdo con la bibliografía, el tratamiento adecuado para este tipo de lesiones es el tratamiento radical.
The treatment plan should include the rehabilitation of the patient to restore functional, anatomical and esthetic capacity. For this purpose, metal reconstruction plates are used to provide greater stability.
En el plan de tratamiento debe incluirse la rehabilitación del paciente para devolverle la capacidad funcional, anatómica y estética. Para este fin se utilizan placas metálicas de reconstrucción que proporcionen mayor estabilidad.
