Saludos a todos mis apreciados lectores de la #colmena, el día de ayer, domingo 20/06/2021, día del padre por cierto; me encontraba de guardia médica en el Hospital Central del estado Guárico; muy nutrida y con un gran aprendizaje, pues me llega una interconsulta por parte de Medicina Interna.
Greetings to all my dear readers of the #colmena, yesterday, Sunday 20/06/2021, Father's Day by the way; I was on medical duty at the Central Hospital of Guárico state; very nourished and with a great learning experience, because I received an interconsultation from Internal Medicine.
Se trataba de una paciente femenina de 37 años de edad, con antecedentes patológicos de base de Paludismo Vivax, diagnosticada hace 3 meses en tratamiento actual con Cloroquina, quién estaba ingresada en dicho servicio bajo diagnóstico: 1.- Infección del sistema nervioso central. 1.1.- Tuberculosis Meníngea. 2.- Paludismo por Plasmodium Vivax.
The patient was a 37-year-old female patient with a history of Vivax malaria, diagnosed 3 months ago and currently treated with chloroquine, who was admitted to the department under diagnosis: 1.- Infection of the central nervous system. 1.1.- Meningeal Tuberculosis. 2.- Plasmodium Vivax Malaria.
Los galenos internistas, notaron que la paciente durante su evolución tórpida, manifestaba mareos. En efecto, al examen físico concuerdan que padecía de un síndrome vertiginoso, y al parecer había afectación del VII y VIII par craneal, es decir, el facial y el vestibulococlear, conclusión que se resume por la sensibilidad en hemicara derecha y la desviación de la comisura al lado derecho.
The internists noticed that the patient, during her torpid evolution, manifested dizziness. In fact, upon physical examination they agreed that she was suffering from vertiginous syndrome, and apparently there was involvement of the VII and VIII cranial pair, that is, the facial and vestibulocochlear, a conclusion that is summarized by the sensitivity in the right hemicara and the deviation of the commissure to the right side.
Muy juicioso, voy a responder ésta solicitud que me emiten. Al llegar a aquella habitación, observo una mujer blanca, ojos húmedos llorosos, que de vez en cuando los cerraba y, apretaba la mandíbula como si algo le dolía.

Es importante resaltar, que el examen físico de Otorrinolaringologia consta de oídos, nariz, boca y cuello. De inmediato examino sus oídos, y me encuentro con un pabellón auricular izquierdo normoimplantado, conducto auditivo externo permeable, membrana timpánica indemne. Pero del otro lado, pabellón auricular derecho normoimplantado, conducto auditivo externo ocupado en su totalidad por un otocerumen de gran magnitud que me impedía visualizar la membrana timpánica. Nariz y Boca dentro de los límites normales, a pesar que medicina interna refería parálisis facial con desviación a la derecha; dato que para mí no fue positivo.

Nose and mouth within normal limits, although internal medicine referred to facial paralysis with deviation to the right; a fact that was not positive for me.
Intento retirar el otocerumen, pero el mismo estaba contactado, es decir, duro y parecía adherido a la mucosa. Aquello costó retirarlo, sin embargo tras diferentes maniobras lo logré, pudiendo visualizar aquella membrana timpánica indemne o sea, sin lesión. Era evidente, que aquel examen físico no me arrojó ningún dato importante, que me hiciera pensar en el por qué ella presentaba aquel vértigo.
I tried to remove the otocerumen, but it was contacted, that is to say, hard and seemed to be adhered to the mucosa. It was difficult to remove it, but after several maneuvers I managed to do it, and I could visualize that tympanic membrane undamaged, that is to say, without lesion. It was evident that that physical examination did not give me any important data that would make me think about why she presented that vertigo.
Procedo a interrogar, y manifiesta que todo empezó el 24 de marzo del presente año, cuando posterior a sus limpiezas de oído con hisopos, comienza a manifestarse mareos, luego se agregó un sonido en el oído derecho como un pito, luego nauseas, vómitos que la llevó a la postración. Visitó muchos médicos pero entre tantos tratamientos, ninguno le hizo efecto, por lo que acudió al hospital 3 meses después y deciden ingresarla.
I proceed to question her, and she states that everything started on March 24 of this year, when after her ear cleaning with swabs, she began to feel dizzy, then a sound in her right ear like a whistle was added, then nausea, vomiting, which led her to prostration. She visited many doctors but among so many treatments, none of them had any effect, so she went to the hospital 3 months later and they decided to admit her.
Finalmente, le digo que no encuentro causa externa a su vértigo, y que aquella parálisis facial estaba superada, quizás, ese pudiese ser un problema, la afectación del nervio facial. Pero algo me llamó la atención, me indicó que no tenía ya el pito; esa presión del oído desapareció al sacar el tapón de cerumen, y que volteaba lentamente al lado derecho sin marearse, cosa que antes no podía hacer.
Finally, I told him that I could not find any external cause for his vertigo, and that the facial paralysis had been overcome, perhaps that could be a problem, the affectation of the facial nerve. But something caught my attention, he indicated that he no longer had the whistle; that ear pressure disappeared when the earwax plug was removed, and that he turned slowly to the right side without getting dizzy, something he could not do before.
Entonces, realizo maniobras vestibulares como Dix-Hallpike, Romberg y Barany, las cuales fueron negativas. Aquello me hizo consultar otro otorrinolaringólogo, creía que en algo yo estaba fallando; pero después de varios análisis se concluyó, que aquel tapón de cerumen me estaba ocasionando todo un conglomerado de signos y síntomas que se manifestaban en la paciente.
Then, I performed vestibular maneuvers such as Dix-Hallpike, Romberg and Barany, which were negative. That made me consult another otorhinolaryngologist, I believed that I was failing in something; but after several analyses it was concluded that the cerumen plug was causing a whole conglomerate of signs and symptoms that manifested in the patient.
Complicaciones como hipoacusia (disminución de la audición), sensación de taponamiento dentro del oído (que se modifica con la masticación), picor, acúfenos (zumbidos o pito en el oído), autofonía (cuando la persona refiere que oye su propia voz al hablar), son las principales manifestaciones descritas en las diferentes bibliografías. Pero en la práctica, estaba ante la presencia de una variante.
Complications such as hypoacusis (hearing loss), sensation of plugging inside the ear (which is modified by chewing), itching, tinnitus (ringing or whistling in the ear), autophony (when the person refers that he hears his own voice when speaking), are the main manifestations described in the different bibliographies. But in practice, there was a variant.
Aquel taponamiento de cera, estaba tan impactado que había producido en la paciente, otodinia (dolor en el oído), vértigo manifestado en crisis aguda (inestabilidad, nauseas, vómitos, temblor al mover la cabeza) e incluso había inducido a la parálisis facial, pues sus otros diagnósticos no tienen nada que ver con la parte ótica.
That wax plugging was so impacted that it had produced in the patient otodynia (pain in the ear), vertigo manifested in acute crisis (instability, nausea, vomiting, tremor when moving the head) and even induced facial paralysis, since her other diagnoses have nothing to do with the otic part.
Un nuevo descubrimiento había ocurrido antes mis ojos. Ahora la paciente sabía que no podía volver a limpiarse jamás los oídos con hisopos, pues al hacerlo, en vez de limpiar lo que hace, es empujar más la cera hacia la membrana timpánica, ocasionando que esta se vaya almacenando, y con el tiempo se endurece y se adhiere a la piel del conducto, y si roza la membrana timpánica, se desencadenan unas series de vibraciones desorganizadas que provocan toda la sintomatología antes descrita.
A new discovery had occurred before my eyes. Now the patient knew that she could never again clean her ears with swabs, because by doing so, instead of cleaning, what it does is to push more wax towards the tympanic membrane, causing it to be stored, and over time it hardens and adheres to the skin of the duct, and if it rubs the tympanic membrane, it triggers a series of disorganized vibrations that cause all the symptoms described above.
Aquel caso clínico, alarmó a diferentes especialistas tanto internistas como otorrinolaringólogos, a tal punto, que el próximo viernes 25 de junio, será discutido ante toda una junta médica especializada y de residentes, cuyo ponente seré yo, dado que, fue un descubrimiento llevado a cabo en mi guardia. Mientras tanto, y por precaución, la paciente quedó medicada por mí a base de esteroides, analgésicos, antivertiginosos, relajantes musculares y vasodilatadores, para terminar de cerrar el capítulo que corresponde a su problema del oído.
That clinical case alarmed different specialists, both internists and otorhinolaryngologists, to such an extent that next Friday, June 25, it will be discussed before a specialized medical board and residents, whose speaker will be me, since it was a discovery made on my watch. In the meantime, and as a precaution, the patient was medicated by me with steroids, analgesics, antivertiginous, muscle relaxants and vasodilators, to finish closing the chapter that corresponds to her ear problem.
Moraleja: Los oídos no se deben limpiar con hisopo ante ninguna circunstancia, solo puede ingresar la yema del dedo índice y retirar lo superficial del pabellón auricular, el resto, déjaselo al especialista en el área, nosotros con mucho amor, placer y satisfacción, retiramos con pinzas y equipos especializados esos restos, que por alguna u otra ocasión quedan atrapados en tu conducto auditivo. El hisopo, solo fue elaborado con la intensión de que te compliques, y debas asistir a la consulta privada otorrinolaringológica, para darle de comer al especialista.
Moral: The ears should not be cleaned with a swab under any circumstances, you can only enter the tip of the index finger and remove the surface of the pinna, the rest, leave it to the specialist in the area, we with love, pleasure and satisfaction, we remove with tweezers and specialized equipment those remains, that for some reason or another are trapped in your ear canal. The swab, was only elaborated with the intention that you complicate yourself, and you must attend the private otolaryngological consultation, to feed the specialist.
