Saludos mis apreciados lectores, como parte de mi aporte a la comunidad, hoy les vengo a hablar de un caso grave que no muy frecuente llega a la consulta de otorrinolaringología, pero que, los pocos que llegan, han reportado daños irreparables posterior a su patología.

El pasado viernes 30 de julio, me llegó un masculino de 34 años de edad, con antecedentes de otitis media de larga data, el cual ha tomado diferentes tratamientos caseros pero ninguno antibiótico, sin conseguir mejoría en ninguno. Al examen físico del oído pude visualizar una masa blanquecina que reposaba sobre su membrana timpánica perforada.
Last Friday, July 30, a 34-year-old male patient came to me with a history of long-standing otitis media, who had taken different home treatments but none of them antibiotics, without improvement in any of them. On physical examination of the ear I could visualize a whitish mass resting on his perforated tympanic membrane.
Tristemente le comenté, estamos ante la presencia de un colesteatoma, el cual es un tumor perlado compuesto por, células inflamatorias crónicas y tejido escamoso estratificado que proviene del hueso temporal; es decir, contiene piel del tipo epidérmico que se enrolla entre sí originando un tumor blanquecino el cual empeora el cuadro.
Sadly, I told him, we are in the presence of a cholesteatoma, which is a pearly tumor composed of chronic inflammatory cells and stratified squamous tissue that comes from the temporal bone; that is to say, it contains epidermal type skin that is rolled together originating a whitish tumor which worsens the picture.
Este tumor, va creciendo a tal punto que llega a desaparecer la cadena de huesecillos, como el martillo, yunque y estribo, originando una acusia (ausencia de la audición) en el oído afectado. Aunque eso no es todo, el dolor, la cefalea, el vértigo y la salida de secreción purulenta, fétida y verdosa que emite de él, no se compara con otro patología, lo que obliga al paciente a buscar al especialista de forma inmediata.
This tumor grows to such an extent that the ossicles chain disappears, such as the hammer, anvil and stirrup, causing an acusia (absence of hearing) in the affected ear. Although that is not all, the pain, the headache, the vertigo and the purulent, fetid and greenish secretion emitted from it, is not comparable to any other pathology, which forces the patient to look for a specialist immediately.
Existen diferentes formas de evaluar o explorar esta enfermedad, entre las pruebas histológicas, las imagenologías, y de forma indispensable, los cultivos para saber que bacteria está presente en la infección y qué antibiótico es el adecuado para iniciar tratamiento de inmediato; no obstante, se debe pensar en la intervención quirúrgica rápida para así frenar las complicaciones pésimas que este tumor deja.
There are different ways to evaluate or explore this disease, including histological tests, imaging, and, indispensably, cultures to know which bacteria are present in the infection and which antibiotic is the appropriate one to start treatment immediately; however, quick surgical intervention should be considered in order to stop the terrible complications that this tumor leaves behind.
Aquel paciente quedó un tanto deprimido, sin embargo tiene la fe más grande en Cristo y en especial la confianza en nosotros de que su caso será resuelto y logrará salir ileso de esta prueba que le trae la vida. Como enseñanza se llevó, que jamás volverá a echar gotas de plantas (orégano orejón), gotas de leche materna, agua oxigenada, etc., para mejorar sus síntomas, de ahora en adelante comenzará a buscar al especialista otorrinolaringólogo para tratar sus afecciones.
That patient was somewhat depressed, however, he has the greatest faith in Christ and especially the confidence in us that his case will be solved and he will be able to come out unscathed from this trial that life brings him. As a lesson he took with him, that he will never again use drops of plants (oregano orejon), drops of mother's milk, hydrogen peroxide, etc., to improve his symptoms, from now on he will begin to look for a specialist otolaryngologist to treat his affections.
