En esta oportunidad, les compartiré un síntoma que suele presentarse de dos maneras en los pacientes depresivos: el trastorno del sueño. Lo primero que debemos saber son las dos funciones fundamentales del sueño: proporcionar el descanso de todos los órganos del cuerpo y permitir al cerebro que organice toda la información recibida durante el día. Cuando algún trastorno del sueño aparece, genera en el paciente una alteración en sus hábitos relacionados con el dormir. A la depresión se le asocia dos tipos de alteraciones del sueño: **el insomnio y la hipersomnia.** El más común, es el insomnio, que es la dificultad para conciliar el sueño. Para definir los tipos de insomnio, quiero citar al *“DSM-IV Manual diagnóstico y estadístico de los trastornos mentales”:* >*(p. ej., despertarse durante la noche y tener problemas para volver a dormirse) o el insomnio tardío (p. ej., despertarse demasiado pronto y ser incapaz de volver a dormirse). También se puede presentar un insomnio inicial (problemas para conciliar el sueño).* En mi caso, a pesar de que mi depresión fue bastante fuerte, no sufrí de insomnio. De hecho solo recuerdo dos noches donde no pude conciliar el sueño. Tenía muchísima ansiedad y no podía quedarme dormido y aunque al final, pude dormir, a las dos o tres horas me desperté (a eso de las 2:00 a. m.) y no pude dormir más. Fue de las noches más largas que he vivido. Y la recuerdo con tanta nitidez porque nunca he tenido ese problema, ni antes, durante, ni después de la depresión.

*In this opportunity, I will share with you a symptom that usually presents itself in two ways in depressive patients: sleep disorder. The first thing to know is the two fundamental functions of sleep: to provide rest for all the organs of the body and to allow the brain to organize all the information received during the day. When a sleep disorder appears, it generates an alteration in the patient's sleeping habits. Two types of sleep disorders are associated with depression: **insomnia and hypersomnia**.* *The most common is insomnia, which is the difficulty in falling asleep. To define the types of insomnia, I would like to quote from the **"DSM-IV Diagnostic and Statistical Manual of Mental Disorders":**.* >*(e.g., waking up during the night and having trouble getting back to sleep) or late insomnia (e.g., waking up too early and being unable to get back to sleep). Initial insomnia (trouble falling asleep) can also occur.* *In my case, although my depression was quite severe, I did not suffer from insomnia. In fact, I only remember two nights where I could not fall asleep. I had a lot of anxiety and I could not fall asleep and although in the end, I was able to sleep, after two or three hours I woke up (around 2:00 a.m.) and I could not sleep anymore. It was one of the longest nights I have ever lived. And I remember it so clearly because I have never had that problem before, during or after the depression.*

Mi situación fue atípica en cuanto a trastornos del sueño. Yo sufrí de exceso de este, o lo que es lo mismo, hipersomnia. Son episodios de sueño prolongado nocturno o de aumento del sueño diurno. Yo dormía muchísimo. Me acostaba temprano, me levantaba relativamente temprano, ya que siempre he sido madrugador. Desayunaba y a la cama de nuevo. No a reposar, sino a dormir. Al medio día, almorzar y de nuevo a la cama. Era un deseo exacerbado de acostarme y dormir. En la hipersomnia, el sueño tiende a ser continuo, pero no reparador. Cito de nuevo al *“DSM-IV Manual diagnóstico y estadístico de los trastornos mentales”*: >Los individuos con este trastorno concilian el sueño muy rápidamente y la eficacia de éste es buena, pero pueden tener dificultades a la hora de levantarse por la mañana, sintiéndose algunas veces confusos, irritables o atáxicos* (dificultad para coordinar movimientos). Esto último me pasaba mucho. Recuerdo una época de esta enfermedad donde presenté momentos en que me iba de lado o caminaba muy lento y torpemente. Es importante resaltar que algunas veces el trastorno del sueño es la razón por la que muchas personas acuden a un médico, especialmente cuando presentan insomnio.

*My situation was atypical in terms of sleep disorders. I suffered from excessive sleep, or hypersomnia. These are episodes of prolonged nighttime sleep or increased daytime sleep. I slept a lot. I went to bed early, got up relatively early, as I have always been an early riser. I would have breakfast and go to bed again. Not to rest, but to sleep. At noon, lunch and back to bed. It was an exacerbated desire to lie down and sleep. In hypersomnia, sleep tends to be continuous, but not restful. I quote again from the **"DSM-IV Diagnostic and Statistical Manual of Mental Disorders"**:* >*Individuals with this disorder fall asleep very quickly and sleep efficiency is good, but they may have difficulty getting up in the morning, sometimes feeling confused, irritable, or ataxic* (difficulty coordinating movements).* *The latter happened to me a lot. I remember a time during this disease when I had moments when I would go sideways or walk very slowly and awkwardly.* *It is important to emphasize that sometimes the sleep disorder is the reason why many people go to a doctor, especially when they have insomnia.*

La hipersomnia fue algo que fui superando tratando de ser más activo y evitando dormir. A medida que iba mejorando otros síntomas como la [anhedonia](https://ecency.com/hive-131951/@garybilbao/esp-eng-symptoms-of-depression), el deseo de dormir iba perdiendo fuerza. Tenía más motivación y razones para levantarme en las mañanas o al menos no dormir tanto en el día. Para tratar casos de hipersomnia, aportaré algunas recomendaciones encontradas en el sitio **"Muy Salud"** y su artículo [Hipersomnia: síntomas, causas y tratamientos](https://muysalud.com/enfermedades/hipersomnia-sintomas-causas-tratamiento/). • Se suele incluir la prescripción de estimulantes. La idea es mantenernos despiertos y este tipo de medicamentos cumplen esta función. • Un psicólogo puede guiarnos en la aplicación de técnicas y medidas propias de la [higiene del sueño](https://lamenteesmaravillosa.com/aprender-a-dormir-bien-higiene-del-sueno/#:~:text=La%20higiene%20del%20sue%C3%B1o%20se,mantenga%20activo%20durante%20el%20d%C3%ADa). • Modificaciones en la dieta o la inclusión de estimulantes naturales. • Finalmente, evitar el consumo de cafeína y alcohol también es recomendable en estos casos. --- >*Because I did not suffer from insomnia, I cannot give a testimony that can reveal how I was able to cope with this symptom. Dos tipos de tratamientos suelen ser los más eficaces para combatir el insomnio. **• Terapia cognitivo-conductual** Este tratamiento psicológico para el insomnio se enfoca en identificar las ansiedades que las personas tienen y luego reemplazar estas ansiedades con creencias y actitudes más saludables. Sus estrategias incluyen: 🔸 **Educación e higiene del sueño:** educar sobre patrones de sueño y hábitos saludables, por ejemplo, establecer una hora constante para acostarse y despertarse. 🔸 **Control de estímulos:** eliminar los factores que condicionan su mente para resistir el sueño, por ej., acostarse solo cuando se sienta cansado, usar la cama solo para dormir y tener relaciones sexuales. 🔸 **Restricción y compresión del sueño:** la restricción del sueño implica una reducción brusca del tiempo en la cama, mientras que la compresión del sueño es un proceso más gradual. 🔸 **Relajación:** incluye ejercicios de respiración, relajación muscular, meditación y biorretroalimentación. * Tratamiento farmacológico del insomnio (medicamentos) Los hipnóticos, pueden funcionar de diferentes maneras. Algunas afectan las áreas del cerebro que controlan el estado de alerta. Otras contienen medicamentos que se usan para tratar otras afecciones, pero causan somnolencia como efecto secundario.
