wrong size / weight / format OR sloppy lot numbers. There are also 2 versions, one from 8/17 and one from 9/03 where the indentation is slightly different.

Yes. Some of them stick mailing label stickers with the Lot#s on there, but most I've seen IRL have been doctor scribble.

There is a database of lot#s to give them a clue, but otherwise they don't have a right to view your personal medical record. Employers can truly do nothing, even if they find out you faked the card. Only real consequences come from trying to defraud federal agencies, like the DoT when travelling.

So, here's how you make a legit card.
Search on duckduckgo for the 09/03 version of MLS-319813_r. You can find a PDF if you dig enough. Print double sided 3x4 inch on 80lb - 110lb cardstock. Make sure you get the tiny text below the CDC logo in good resolution.
Fill in your name, etc. For lot numbers go to the VAERS website and download the VAERSdata.csv and VAERSvaccine.csv files. VEARSdata.csv has a list of vaccine reaction cases, so filter by your state and date and write down the Case ID. VAERSvaccine.csv has all the lot numbers for each case ID, so cross reference from the other file to get your legit lot number. Its easiest just to find a J&J single jab one.
After you have the card, go test it out by getting free donuts or other carrot-on-stick ploys they have around your area. I've used it for my employer. My dad used his to evade quarantine in Hawaii.
