It is usually given intravenously using an IV-line, the sole purpose is to bombard those nasty methemoglobin that have decided to be stingy with oxygen and refusing to give it out by basically reducing their carrying power for oxygen. So essentially their superpower which is ferric is being reduced by methylene blue to ferrous which is more willing to give out oxygen.
